OFFICIAL RULES AND REGULATIONS – Atlanta Falcons 2010 Season Ticket Giveaway

HOW TO ENTER: The Sweepstakes begins on November 8, 2009 and ends on December 31, 2009 (otherwise known as the "Entry Period"). There are three ways to enter.

All patients who complete their LASIK procedure during the Entry Period will be automatically entered into the drawing or you may enter by sending a hand printed or typed 3”x5” postcard (no envelopes) with your full name, age, complete address and mail it via U.S. Mail to:  Woolfson Eye Institute, Away Game Sweepstakes Entry, P.O. Box 89132, Atlanta, Georgia 30312.  All mailed entries must be postmarked by 10/31/2009 in order to be considered or by going online to www.woolfsoneye.com/falcons_offer.aspx.  ONLY the winning entries will be contacted.

ELIGIBILITY: The Atlanta Falcons 2010 Season Ticket Giveaway is open to legal residents of the fifty (50) United States and the District of Columbia who are twenty-one (21) years of age or older as of the date of entry. Employees (and members of their immediate families or household) of Woolfson Eye Institute (the "Sponsor"), its franchisees, or of any of their respective parent companies, subsidiaries, affiliates, advertising and promotional agencies, and/or sweepstakes promotional partners, and any other person or entity involved in the design, execution or judging of the Sweepstakes are not eligible.

Sponsor is not responsible for misdirected, late, incomplete, illegible, or lost entries, or any unforeseen circumstances, which may limit an individual's ability to participate in the Sweepstakes. All entries become the property of the Sponsor and will not be returned. Limit one entry per person. Mailed entries must be sent separately and bare sufficient postage.

WINNER SELECTION: The odds of winning depend upon the total number of eligible entries received during the Entry Period. Prize Winners will be selected during a random drawing conducted on or about 1/4/2010 by T.Vanderbilt, a marketing organization, whose decisions will be final.  Prize winners will be notified via telephone and U.S. mail within three (3) days after the drawing date and must complete an Affidavit of Eligibility and Liability Release, which includes the winner's tax identification number for tax reporting purposes. Non-compliance may result in disqualification and selection of an alternate winner. Return of any prize notification as undeliverable may result in disqualification and selection of an alternate winner solely at the Sponsor's discretion. Sponsor will use reasonable efforts to select an alternate winner but in the event one cannot be reasonably determined, such prize will not be awarded.

PRIZE:  2010 Season Ticket.  Approximate Retail Value of the Prize is $1100.

GENERAL CONDITIONS: Contest offered to eligible participants, but void where prohibited or restricted by law. By submitting an entry, you agree to release Woolfson Eye Institute, its franchisees, all participating retailers, advertising agencies and their respective officers, directors, employees, agents, parent companies, affiliates and subsidiaries from liability of any kind or nature for any loss, claims, damages, or injuries of any kind associated with participation in sweepstakes or acceptance, possession, participation in or use of any prizes. You accept and agree to be bound by these rules and the decisions of the Sponsor relating to participation in Sweepstakes, selection of winners and awarding of prizes, whose decisions are final. Acceptance of prize constitutes permission to use winner's name, biographical information, and/or likeness for purposes of advertising and promotion without further compensation, except in Tennessee and where prohibited by law. Any Sweepstakes materials (including without limitation, offer, rules and announcement of winners) containing production, printing or typographical errors or obtained outside authorized legitimate channels may be declared void at the discretion of Sponsor; and liability is limited to replacement of such materials. Recipient of any such materials agrees to release Aaron Rents, Inc., its franchisees, all participating retailers, contest promoter, advertising agencies and their respective officers, directors, employees, agents, parent companies, affiliates and subsidiaries from any and all losses, claims or damages that may result in connection with any such materials. The Sponsor expressly disclaims any responsibility, and entrants agree to hold Sponsor, harmless for liability for injury or loss to any person or property relating to participation in this Sweepstakes or the delivery and/or subsequent misuse of any of the prizes awarded.

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT IMPROVE CHANCES OF WINNING.
